More must be done to get black, Asian and ethnic minority people to participate in breast cancer trials, experts have said, as they warned that people from these backgrounds have been under-represented in previous studies. Medics said that they want research into the disease to be “relevant to people we see in the clinic”. Experts said that there is a “broad misperception” that black women “don’t suffer as much from breast cancer” which can result in the perception that “cancer is a white person’s disease”. They are also more likely to develop more aggressive cancer and be diagnosed when their cancer is at a more advanced stage. It comes as the NHS Race and Health Observatory launched a new campaign alongside Macmillan Cancer Support to improve diversity in breast cancer clinical trials. The project, which is being supported by Roche, aims to raise awareness of the lack of diversity in clinical studies, improve communications and provide longer term support to patients. Specialist nurses will be provided at two major cancer hubs – Bart’s Health NHS Trust in London and The Christie NHS Foundation Trust in Manchester – to help guide patients through the process. Men, who account for 1% of breast cancer patients in the UK, are also being included. The NHS Race and Health Observatory said that there are “multiple barriers” around the recruitment, communication and retention of black, Asian and ethnic minority patients in clinical trials. It said that data show that people from an ethnic minority background are poorly under-represented in many clinical trials. And the Caribbean African Health Network said that there has been a “disengagement” in research “as a result of mistrust”. Jasmin David, a 53-year-old breast cancer patient from Fallowfield in south Manchester, took part in a clinical trial which saved her life and is encouraging others to do the same. Two years after her initial diagnosis and treatment at The Christie she was told that the cancer had come back and had spread to her lungs, lymph nodes and chest bone. The mother-of-two was told she had less than a year to live but was also offered the opportunity to take part in a clinical trial at the National Institute for Health and Care Research Manchester Clinical Research Facility. Ms David is now cancer-free. “If I hadn’t gone on the trial at The Christie, I wouldn’t be here today,” she said. “I have two children and now I get to be there for them as they grow up. “Research gave me a second chance and life and I’m relishing every second of it. “I want everyone, no matter their ethnicity, to have equal access to clinical trials, so I’m glad that this important piece of work is being done. I hope that by sharing my story I can inspire more women like me to come forward and take part in clinical trials.” Dr Habib Naqvi, chief executive of the NHS Race and Health Observatory, said: “We are pleased to announce this partnership and our joint commitment to ensuring inclusion and representation in future breast cancer trials. “We believe that when targeted, culturally sensitive interventions and communications are put in place, under-represented groups can be successfully recruited into clinical trials.” He added: “There is a broad misperception that black women don’t suffer as much from breast cancer or it does not run in their family history. This can result in the perception that cancer is a white person’s disease. “We want this pilot to encourage women at risk, those already diagnosed and individuals undergoing post treatment to come forward and share their experiences and get the information needed.” Professor Richard Simcock, chief medical officer at Macmillan Cancer Support, added: “As a Breast Cancer Oncologist I want to know that research is relevant to the people we see in clinic. Historically that has not been the case. “I’m delighted that Macmillan can support this project to ensure that future evidence from clinical trials is representative and inclusive.” Charles Kwaku-Odoi, chief executive of the Caribbean African Health Network, said: “Across the black community there is an undoubted legacy of disengagement in research and most certainly clinical trials that stems back decades as a result of mistrust. “This has not served us well because it leads to a lack of appropriate interventions that perpetuate the grave health inequalities in breast cancer care. “This partnership approach to build solutions to improve engagement in clinical trials in breast cancer treatment and care is very much welcomed. Are Red Bull now the most successful F1 team ever – and how long can this dominance last?
At the moment it seems a grand prix does not go by without Red Bull – and their peerless linchpin Max Verstappen – setting milestones and breaking records. Such is their dominance in Formula 1 right now, winning 18 of the last 19 races, the double-symphony of the Dutch and Austrian national anthems on the podium is as much a regularity these days as five red lights going out to start the race. But Canada really did take the biscuit. Red Bull’s 100th race victory; Max Verstappen’s 41st grand prix win, equalling Ayrton Senna’s record; Verstappen brought up 250 consecutive laps led, dating back to Miami six weeks ago; Red Bull’s 10th consecutive victory, including Abu Dhabi at the end of last year. It was notable who was on the podium, too, celebrating Verstappen’s 20th win in his last 27 races. Adrian Newey, Red Bull’s mastermind-in-chief, who was unofficially celebrating his 200thF1 race win involvement, dating back to his first in Mexico 32 years ago. The man extolled, appropriately, with engineering the team’s latest RB missile on four wheels. By this season’s standards, Verstappen’s win in Montreal was far from a cruise. Qualifying on Saturday was tricky in wet-dry-wet conditions. But Max prevailed. An early safety car bunched the pack up on Sunday. But Max prevailed. Tyre wear was an issue in the cockpit with Fernando Alonso and Lewis Hamilton chasing; not on the pit wall who again oozed perfectibility. Again, Max prevailed. No 100 is a significant landmark for the team. An “incredible achievement”, as Christian Horner put it, becoming the fifth team to reach such a figure. When he spearheaded Red Bull’s involvement by taking over Jaguar in 2004, surely even he could not have forecasted such periods of dominance, up against the historic titans of world motorsport. Red Bull have won 28.1% of all the races they’ve entered in the sport as they hit a century. By way of comparison, Ferrari have won 242 races in 1060 – a hit-rate of 22.8%. McLaren’s is lower at 19.6%; Williams’ fewer still at 14.4%. But Mercedes at 44.8% beat the lot. Such was their near-decade of utter dominance – eight constructors’ titles in a row from 2014-2021 – Red Bull have some way to go before their percentage hit-rate matches the Silver Arrows. Currently, the difference is 16.7%. MOST WINS IN FORMULA 1 – 100 CLUB 1) Ferrari – 242 (22.8% hit-rate) 2) McLaren – 183 (19.6% hit-rate) 3) Mercedes – 125 (44.8% hit-rate) 4) Williams – 114 (14.4% hit-rate) 5) Red Bull – 100 (28.1% hit-rate) There are contentious points. For example, included in Mercedes’ tally is the nine wins from 1954-55, technically a different entity to the current works team. Yet on the whole, the difference is stark. But not insurmountable. It’s important to note this with caution. Predicting dynasties in sport is always a dangerous game: jump the gun on a Formula 1 forecast and the usual unpredictability of natural drama and human emotion can come back to bite you. Ferrari, for instance, have won just one drivers’ title in 16 years since their five-on-the-spin with Michael Schumacher in the early noughties. However, the outlook is particularly shiny at Milton Keynes. Red Bull have unequivocally nailed the current “ground-effect” regulations, not least due to Newey’s expertise in the area. This year is virtually sewn-up, with focus already switching to 2024 when they will be the heavy favourites once more. The next phase of regulations are not enforced until 2026. By that point, with Newey having recently signed an extension to stay, Red Bull could have secured another three world titles. It could be another 50 or so wins by that point, such is the current trajectory. And from then, the launch of their own powertrains division comes to fruition, with American giant Ford as a partner. In the constantly-shifting arms race that is Formula 1, Aston Martin, Mercedes, Ferrari and the rest – including new teams such as Audi – may well have something to say about that. Indeed, Red Bull have recently lost a key figure in engineer Rob Marshall, who joins McLaren next year. Dan Fallows similarly joined Aston Martin last year. Newey’s omnipresence is the unignorable advantage. Yet as the sun set in Canada on another victory, did the 64-year-old give Red Bull a scare? And the rest of the paddock a glimmer of hope? “My career can’t go on forever,” he said, post-podium. “As long as the team wants me and I keep enjoying it I’ll keep going. “But realistically it’s on a countdown.” When that time will be, only he knows. What is not in question, however, is the absolute supremacy of this current Red Bull machine. From top-to-bottom, they now set the benchmark. The statistics of the past allude to that; the projection for the future indicate there could be plenty more to come. World Mental Health Day: 5 ways to beat anxiety and change your life
Anxiety is something that touches most of us, to a greater or lesser extent. Although it may just be a temporary blip for some people, for many, anxiety is a diagnosed condition that blights their lives. Indeed, Mental Health UK estimates more than one in 10 people in the UK are living with an anxiety disorder – that’s over eight million of us. And World Mental Health Day (October 10) is a chance to address the topic, agrees clinical psychologist Dr Kirren Schnack – author of the new book Ten Times Calmer: Beat Anxiety And Change Your Life. “It’s a reminder that mental wellbeing is just as important as physical health,” she says, pointing out that anxiety disorders are among the most widespread mental health issues globally. “Clinically, I’ve seen more and more people coming in with anxiety issue. The pandemic has played a significant role in this surge, introducing unprecedented levels of uncertainty, fear, and social isolation. At the same time, global economic instability and job losses have intensified stress and anxiety for many,” Schnack adds. “The widespread use of digital technology and social media has exposed people to a constant stream of alarming news and comparison-driven content that also feeds anxiety.” Schnack explains that anxiety disorders include generalised anxiety disorder (GAD), which causes chronic worry and hinders decision-making; health anxiety, where there’s an excessive preoccupation with diseases and illness; panic disorder, which is marked by sudden overwhelming panic attacks; and social anxiety, which leads to avoidance, isolation and low confidence. “They can blight daily life through physical symptoms, cognitive impairment, social isolation, disrupted routines, and emotional distress,” she says. Schnack suggests these five ways to combat anxiety problems and find calm… 1. Calm your stressed nervous system Anxiety places significant strain on the nervous system, Schnack explains, but there are simple daily practices that can help alleviate this stress. One is focusing on breathing, extending your exhale longer than your inhale. To do this, Schnack says you need to inhale gently through your nose, pause for a few seconds, then exhale slowly and deeply, ensuring an extended exhale. “A longer exhale helps by increasing the activation of the body’s parasympathetic nervous system, which counteracts stress hormones, lowers heart rate, and reduces overall physiological stress,” she says. She suggests around three to five of the breaths are repeated on multiple occasions throughout the day, and stresses that consistency is key. “Recognise that if anxiety has been a longstanding presence, it will take time and patience to restore your nervous system’s sense of safety and stability.” 2. Address anxious thoughts Schnack explains that anxiety often generates anxious thoughts that can seem like undeniable facts that are often dwelled on and reacted to, thus intensifying anxiety. “To break free from this cycle, it’s crucial to consciously confront and process these anxious thoughts,” she says. To do this, write a simple record, noting the date of your anxious prediction, describing what it was, and then, after a specified time like a day or two, return to what you’ve written and confirm whether the prediction was accurate by marking it as ‘yes’ or ‘no’. When the prediction doesn’t materialise, make a note of the actual outcome. Revisit the record, especially during anxious moments, and count the yes or no entries to reflect on your progress. “This practice trains your mind to adopt a broader, less anxious perspective,” explains Schnack. 3. Stop over-focusing on anxiety“People struggling with anxiety often find it challenging to shift their attention away from anxiety symptoms, leading to a persistent preoccupation that can worsen their problems,” says Schnack, who explains that this preoccupation can lead to excessive self-analysis, fixation on physical sensations or repetitive thoughts, and being hypervigilant. To shift your attention away from overwhelming anxiety, Schnack suggests trying this exercise: choose a colour, such as blue, and actively seek out blue objects in your surroundings. Name them out loud, and count them as you move around, possibly describing the item too. “Strive to maintain this focus for a few minutes, switching to different colours if necessary. Again, consistency is crucial, so make an effort to practice this regularly whenever anxiety hijacks your attention,” she advises. 4. Learn to tolerate uncertainty Recognising and addressing uncertainty is vital when dealing with anxiety, Schnack stresses. “You can’t resolve every uncertainty in life – the key lies in enhancing your ability to tolerate uncertainty so you can reduce your anxiety,” she explains. “Being more accepting of the fact that things may not always go as planned or that the future is uncertain can be incredibly liberating.” To help build resilience against uncertainty, Schnack advises people to take a moment to slow down and not immediately engage in reactive behaviour demanded by uncertainty, such as checking, Googling or avoiding things. Instead, she says, note your feelings, observing what uncertainty is doing to your mind and body, and write out an affirmation such as: ‘I’m feeling anxious because I’m uncertain about… My typical response to uncertainty is to engage in… behaviour. I recognise this only worsens my anxiety. My goal is to distance myself from anxiety rather than gravitate towards it. I can sit with the feeling of uncertainty for as long as I can, and build on this time until I get better and better at it.’ 5. Gradually face your fearsConfronting your fears is an effective way to overcome them, observes Schnack, although this can often feel overwhelming. “When you confront your fears, you’re essentially acting in opposition to your anxious thoughts and what anxiety is telling you. This process weakens anxiety’s grip on you, and when the actual outcomes don’t align with your anxious predictions, your mind can naturally adjust its thinking.” She suggests starting with small steps and gradually building up. So, for social anxiety, gradually start by saying a simple sentence or asking a basic closed question, and gradually building up from there. For health anxiety, gradually reduce and eliminate excessive online researching about illness and diseases, starting with small-time durations and then building on that. For panic disorder, gradually spend more time in places you typically avoid, starting with short durations like 30 seconds, then a few minutes, and gradually keep increasing the time. “Remember to repeatedly face your fears to make meaningful progress – it’s not just a one time thing,” adds Schnack.
