This time we don't see exactly what John Kramer's (Tobin Bell's) victim is about to be put through, but the setup gives us a pretty good idea: Medical equipment on a tray and a man strapped to a chair with a portion of his head shaved. Looks like some kind of DIY brain surgery will be the only way out? Ick.
